STUDENT ORGANIZATION ROOM/EVENT REQUEST FORM: Submit this form to request a room reservation for all organization meetings and events as well as for flyer approval. A friendly reminder that only the 2 designated officers listed in your re-registration form may submit this form. Be mindful of the submission dates.

STUDENT REIMBURSEMENT REQUEST FORM:  This form should be completed by students seeking reimbursement from student organization accounts for club related expenses.  It may also be used to request reimbursement from University accounts for approved expenses.  This is an electronic form.  Once submitted, it will be sent to all required parties for approval (club president, treasurer, and faculty advisor).

This form is for new student clubs and organizations. Please fill out and submit this form to register as new student club/organization or if your student club/organization has been inactive for two consecutive trimesters.

This form can be electronically completed when downloaded. This form should be completed anytime an organization officer changes their position or anytime a new individual becomes an officer. Each time, this form must also be signed by the organization Advisor on file.

The Department of Student Affairs is required by State Law to keep a record of solicitation, fundraising contributions, or sales conducted on the Parker University campus, but outside of your members. The time period should include all solicitation in the PAST year. Please complete and upload this form with your Registration or Re-Registration Form EACH trimester.

Speakers who have been previously approved and are still within their approval year will submit the Proposed Presentation Outline Form for review by the Outside Speaker Committee.

*Speakers who will be adjusting will also need to submit a new video demonstration.

For organizations that meet the following criteria:

• The activity or event is sponsored, organized and funded by Parker University.
• The activity or event is located more than twenty-five (25) miles from Parker University
• The activity or event is either:
  • Undertaken, using a vehicle owned, leased, or rented by the institution; or attendance at the activity OR
  • Event is required by a registered student organization and approved in writing by the Dean of Student Affairs.
