STUDENT REIMBURSEMENT REQUEST FORM:  This form should be completed by students seeking reimbursement for University-approved expenses not related to student organizations or clubs.  Student organization reimbursement requests must be submitted on www.ParkerEngage.com

ASSEMBLY CREDIT INQUIRY FORM: This form should be used to inquire about any credits that are missing from a student’s assembly credit report.  Please note that updates will typically only be made for credits earned during the previous trimester.

ROOMMATE FORM:  Roommate Form

For organizations that meet the following criteria:

• The activity or event is sponsored, organized and funded by Parker University.
• The activity or event is located more than twenty-five (25) miles from Parker University
• The activity or event is either:
  • Undertaken, using a vehicle owned, leased, or rented by the institution; or attendance at the activity OR
  • Event is required by a registered student organization and approved in writing by the Dean of Student Affairs.

Parking Regulations and Appeals

NOTE: A copy of the citation must be submitted with appeal form.  Email the completed form along with a copy of the citation to AskStudentAffairs@parker.edu.

Citations must be paid within 10 days of issuance to avoid a $25 late fee.  Appealing does not grant more time to pay.  All citation fees will be reversed if an appeal is granted.
